Donatussin DM Suspension

Pronunciation: FEN-il-EF-rin/DEX-klor-fen-IR-a-meen/DEX-troe-meth-OR-fan
Generic Name: Phenylephrine/Dexchlorpheniramine/Dextromethorphan
Brand Name: Donatussin DM

Donatussin DM Suspension is used for:

Relieving symptoms of sinus congestion, runny nose, sneezing, itchy nose or throat, itchy or watery eyes, and cough due to colds, upper respiratory infections, and allergies. It may also be used for other conditions as determined by your doctor.

Donatussin DM Suspension is a decongestant, antihistamine, and cough suppressant combination. The decongestant works by constricting blood vessels and reducing swelling in the nasal passages. The antihistamine works by blocking the action of histamine, which helps reduce symptoms such as watery eyes and sneezing. The cough suppressant works in the brain to help decrease the cough reflex to reduce a dry cough.

Do NOT use Donatussin DM Suspension if:

  • you are allergic to any ingredient in Donatussin DM Suspension

  • you have severe high blood pressure, severe heart blood vessel disease, rapid heartbeat, or severe heart problems

  • you are unable to urinate or are having an asthma attack

  • you take droxidopa, sodium oxybate (GHB), or you have taken furazolidone or a monoamine oxidase inhibitor (MAOI) (eg, phenelzine) within the last 14 days

dornase alfa Inhalation, oral/nebulization

DOR-nase AL-fa

Commonly used brand name(s)

In the U.S.

  • Pulmozyme

Available Dosage Forms:

  • Solution

Therapeutic Class: Mucolytic

Pharmacologic Class: Mucolytic Enzyme

Uses For dornase alfa

Dornase alfa is used in the management of cystic fibrosis. It is used every day with other cystic fibrosis medicines, especially antibiotics, bronchodilators (medicines that open up narrowed breathing passages), and corticosteroids (cortisone-like medicines).

Cystic fibrosis is a condition in which thick mucus is formed in the lungs and breathing passages. The mucus blocks the airways and increases the chance of lung infections. The infections then cause the mucus to become even thicker, making it more difficult to breathe.

Dornase alfa will not cure cystic fibrosis. However, when it is used every day, it helps make breathing easier and reduces the number of serious lung infections that require treatment with antibiotics.

Dornase alfa is available only with your doctor's prescription.

Before Using dornase alfa

In deciding to use a medicine, the risks of taking the medicine must be weighed against the good it will do. This is a decision you and your doctor will make. For dornase alfa, the following should be considered:

Allergies

Tell your doctor if you have ever had any unusual or allergic reaction to dornase alfa or any other medicines. Also tell your health care professional if you have any other types of allergies, such as to foods, dyes, preservatives, or animals. For non-prescription products, read the label or package ingredients carefully.

Pediatric

Dornase alfa has been studied in children 3 months of age and older and has not been shown to cause different side effects or problems in children than it does in adults, although coughing, runny or stuffy nose, and skin rashes were more common in children 3 months to 5 years of age than in other age groups.

Geriatric

Dornase alfa has not been tested on enough patients 65 years and older. Cystic fibrosis is a disease that usually affects children and young adults. Although there is no specific information comparing use of dornase alfa in the elderly with use in other age groups, dornase alfa is not expected to cause different side effects or problems in older people than it does in younger adults.

Interactions with Medicines

Although certain medicines should not be used together at all, in other cases two different medicines may be used together even if an interaction might occur. In these cases, your doctor may want to change the dose, or other precautions may be necessary. Tell your healthcare professional if you are taking any other prescription or nonprescription (over-the-counter [OTC]) medicine.

Interactions with Food/Tobacco/Alcohol

Certain medicines should not be used at or around the time of eating food or eating certain types of food since interactions may occur. Using alcohol or tobacco with certain medicines may also cause interactions to occur. Discuss with your healthcare professional the use of your medicine with food, alcohol, or tobacco.

Proper Use of dornase alfa

Dornase alfa usually comes with patient instructions. Read them carefully before using dornase alfa.

Dornase alfa is packaged in small plastic containers called ampules. Each ampule contains one full dose of dornase alfa. Do not use an ampule that has already been opened. Also, do not use an ampule of dornase alfa after the expiration date printed on the package.

Do not use dornase alfa solution if it is cloudy or discolored.

Dornase alfa must be used in a nebulizer with a compressor . Only the following nebulizers and compressors should be used with dornase alfa:

  • Hudson T Up-draft II disposable jet nebulizer used with the Pulmo-Aide compressor

  • Marquest Acorn II disposable jet nebulizer used with the Pulmo-Aide compressor

  • Reusable PARI LC Jet+ nebulizer used with the PARI PRONEB compressor

  • Reusable PARI BABY nebulizer used with the PARI PRONEB compressor

  • Reusable Durable Sidestream jet nebulizer with the MOBILAIRE or the Porta-Neb compressor

Your health care professional will help you decide which nebulizer and compressor to use.

It is very important that you use dornase alfa only as directed. Use the mouthpiece provided with the nebulizer. Do not use a face mask with the nebulizer because less medicine will get into your lungs. Patients who are unable to inhale and exhale through the mouth for the entire nebulizer treatment may use the PARI BABY nebulizer with the supplied face mask. Make sure you understand exactly how to use dornase alfa in the nebulizer.

In order to receive the full effects of dornase alfa, you must use it every day as ordered by your doctor. If possible, dornase alfa should be used at about the same time each day. You may notice some improvement in your condition within the first week of treatment. However, some patients may not feel the full effects of dornase alfa for weeks or months.

If you are taking any other medicines for cystic fibrosis, keep taking them as you did before you started using dornase alfa, unless otherwise directed by your doctor. However, do not put any other inhaled medicine in the nebulizer at the same time that you use dornase alfa. Other inhaled medicines may be used in a clean nebulizer before or after your treatment with dornase alfa.

To prepare the nebulizer for use:

  • Wash your hands well with soap and water before putting the nebulizer together and preparing the medicine. This will help prevent infection.

  • Put the nebulizer together only on a clean surface. If dirt or germs get on the nebulizer or in the medicine, they may cause infection.

  • After you put the nebulizer together, test the compressor (following the manufacturer's directions) to make sure it works properly. If you have any questions about this, ask your health care professional.

  • If you are using the Durable Sidestream jet nebulizer with the MOBILAIRE compressor, turn the compressor control knob all the way to the right and then turn on the compressor. The needle on the gauge vibrates between 35 and 45, which is the highest amount of pressure.

To prepare the medicine for use in the nebulizer:

  • Remove one ampule of dornase alfa from the refrigerator. Squeeze the ampule before opening to make sure there are no leaks. Hold the tab at the base of the ampule firmly. Twist off the top of the ampule, but do not squeeze the body of the ampule while doing so.

  • Take the cap off the nebulizer cup. Turn the opened ampule upside down over the cup. Squeeze the ampule gently until all the contents are emptied into the cup. It is very important that you use the full dose of dornase alfa.

  • Replace the cap on the nebulizer cup. Connect the nebulizer and compressor, following the manufacturer's directions.

  • Turn on the compressor. Make sure there is mist coming from the nebulizer.

To use the medicine in the nebulizer:

  • Place the mouthpiece between your teeth and on top of your tongue. Close your lips around the mouthpiece. Be sure that you do not block the air flow with your tongue or teeth.

  • Breathe normally, in and out, through your mouth. Do not breathe through your nose. If you have trouble breathing only through your mouth, use a nose clip.

  • During treatment, moisture may collect in the long connecting tube of the nebulizer. This should be expected. However, if you notice a leak or feel moisture coming from the nebulizer during the treatment, turn off the compressor. Then check to make sure the nebulizer cap is sealed correctly before you continue the treatment.

  • When the nebulizer begins 'spitting' gently tap the nebulizer cup. Continue breathing until the cup is empty or no more mist comes from the nebulizer.

  • If you have to stop the treatment for some reason or if you start coughing during the treatment, turn off the compressor. To begin the treatment again, turn on the compressor and continue as before.

  • The complete treatment usually takes 10 to 15 minutes. Be sure to inhale the full dose of dornase alfa.

After using dornase alfa:

  • Turn off the compressor. Then take apart the nebulizer system.

  • Follow the manufacturer's directions for care and cleaning of your nebulizer and compressor.

Dosing

The dose of dornase alfa will be different for different patients. Follow your doctor's orders or the directions on the label. The following information includes only the average doses of dornase alfa. If your dose is different, do not change it unless your doctor tells you to do so.

The amount of medicine that you take depends on the strength of the medicine. Also, the number of doses you take each day, the time allowed between doses, and the length of time you take the medicine depend on the medical problem for which you are using the medicine.

  • For inhalation dosage form (inhalation solution):
    • For cystic fibrosis:
      • Adults and children 5 years of age and older—The usual dose is 2.5 milligrams (mg) (one ampule), used in a nebulizer once a day for about 10 to 15 minutes. However, your doctor may want you to use dornase alfa two times a day at regularly spaced times.

      • Children 3 months to 5 years of age—The usual dose is 2.5 milligrams (mg) (one ampule), used in a nebulizer once a day for about 10 to 15 minutes.

      • Children up to 3 months of age—Use and dose must be determined by your doctor.

Missed Dose

If you miss a dose of dornase alfa, take it as soon as possible. However, if it is almost time for your next dose, skip the missed dose and go back to your regular dosing schedule. Do not double doses.

Storage

Keep out of the reach of children.

Do not keep outdated medicine or medicine no longer needed.

Store the medicine in the refrigerator in the foil pouches. However, keep the medicine from freezing. Do not leave dornase alfa out of the refrigerator for longer than 24 hours. If an ampule of medicine is left out for longer than this, it should be thrown away and a new ampule should be used.

Precautions While Using dornase alfa

If your condition becomes worse while you are using dornase alfa, check with your doctor.

Hydrocodone is in a group of drugs called narcotic pain relievers.

Acetaminophen is a less potent pain reliever that increases the effects of hydrocodone.

The combination of acetaminophen and hydrocodone is used to relieve moderate to severe pain.

Acetaminophen and hydrocodone may also be used for purposes not listed in this medication guide.

What is the most important information I should know about Dolorex Forte (acetaminophen and hydrocodone)?

Tell your doctor if you have ever had alcoholic liver disease (cirrhosis) or if you drink more than 3 alcoholic beverages per day. You may not be able to take medicine that contains acetaminophen. Do not take more of this medication than is recommended. An overdose of acetaminophen can damage your liver or cause death. Tell your doctor if the medicine seems to stop working as well in relieving your pain. Hydrocodone may be habit-forming and should be used only by the person it was prescribed for. Keep the medication in a secure place where others cannot get to it. Ask a doctor or pharmacist before using any other cold, allergy, pain, or sleep medication. Acetaminophen (sometimes abbreviated as APAP) is contained in many combination medicines. Taking certain products together can cause you to get too much acetaminophen which can lead to a fatal overdose. Check the label to see if a medicine contains acetaminophen or APAP. This medication may impair your thinking or reactions. Be careful if you drive or do anything that requires you to be alert. Avoid drinking alcohol. It may increase your risk of liver damage while taking acetaminophen.

What should I discuss with my healthcare provider before taking Dolorex Forte (acetaminophen and hydrocodone)?

Do not use this medication if you are allergic to acetaminophen (Tylenol) or hydrocodone. Tell your doctor if you have ever had alcoholic liver disease (cirrhosis) or if you drink more than 3 alcoholic beverages per day. You may not be able to take medicine that contains acetaminophen.

Never share acetaminophen and hydrocodone with another person, especially someone with a history of drug abuse or addiction. Keep the medication in a place where others cannot get to it. FDA pregnancy category C. It is not known whether this medication is harmful to an unborn baby, but it could cause breathing problems or addiction/withdrawal symptoms in a newborn. Tell your doctor if you are pregnant or plan to become pregnant while using this medication. Acetaminophen and hydrocodone can pass into breast milk and may harm a nursing baby. Do not use this medication without telling your doctor if you are breast-feeding a baby.

How should I take Dolorex Forte (acetaminophen and hydrocodone)?

Take exactly as prescribed. Never take acetaminophen and hydrocodone in larger amounts, or for longer than recommended by your doctor. An overdose of acetaminophen can damage your liver or cause death.

One acetaminophen and hydrocodone tablet may contain up to 750 mg of acetaminophen. Know the amount of acetaminophen in the specific product you are taking.

Follow the directions on your prescription label. Tell your doctor if the medicine seems to stop working as well in relieving your pain.

Measure liquid medicine with a special dose-measuring spoon or medicine cup, not with a regular table spoon. If you do not have a dose-measuring device, ask your pharmacist for one.

Drink 6 to 8 full glasses of water daily to help prevent constipation while you are taking acetaminophen and hydrocodone. Ask your doctor about ways to increase the fiber in your diet. Do not use a stool softener (laxative) without first asking your doctor. Do not stop using this medicine suddenly after long-term use, or you could have unpleasant withdrawal symptoms. Ask your doctor how to avoid withdrawal symptoms when you stop using acetaminophen and hydrocodone.

Acetaminophen can cause false results with certain lab tests for glucose (sugar) in the urine. Talk to your doctor if you are diabetic and you notice changes in your glucose levels during treatment.

If you need surgery, tell the surgeon ahead of time that you are using acetaminophen and hydrocodone. You may need to stop using the medicine for a short time.

Keep track of the amount of medicine used from each new bottle. Oxycodone is a drug of abuse and you should be aware if anyone is using your medicine improperly or without a prescription.

Always check your bottle to make sure you have received the correct pills (same brand and type) of medicine prescribed by your doctor. Ask the pharmacist if you have any questions about the medicine you receive at the pharmacy.

What happens if I miss a dose?

Since acetaminophen and hydrocodone is taken as needed, you may not be on a dosing schedule. If you are taking the medication regularly, take the missed dose as soon as you remember. Skip the missed dose if it is almost time for your next scheduled dose. Do not use extra medicine to make up the missed dose.

What happens if I overdose?

Seek emergency medical attention or call the Poison Help line at 1-800-222-1222. An overdose of acetaminophen and hydrocodone can be fatal.

The first signs of an acetaminophen overdose include loss of appetite, nausea, vomiting, stomach pain, sweating, and confusion or weakness. Later symptoms may include pain in your upper stomach, dark urine, and yellowing of your skin or the whites of your eyes.

Overdose symptoms may also include extreme drowsiness, pinpoint pupils, cold and clammy skin, muscle weakness, fainting, weak pulse, slow heart rate, coma, blue lips, shallow breathing, or no breathing

Proper Use of acetaminophen

This section provides information on the proper use of a number of products that contain acetaminophen. It may not be specific to Dolono. Please read with care.

Take this medicine only as directed by your doctor. Do not take more of it, do not take it more often, and do not take it for a longer time than your doctor ordered. If too much of this medicine is taken for a long time, it may cause an overdosage. Liver damage can occur if large amounts of acetaminophen are taken for a long time.

Carefully check the labels of all other medicines you are using, because they may also contain acetaminophen. It is not safe to use more than 4 grams (4,000 milligrams) of acetaminophen in one day (24 hours).

You may take this medicine with or without food.

If you are taking this medicine without the advice of your doctor, carefully read and follow the drug facts label and dosing instructions on the medicine package. This is to avoid confusion and dosing errors. Ask your doctor if you have any questions.

For patients using the oral liquid with syringe (e.g. Little Fevers®):

Ask your healthcare professional how you should dispose of any medicine you do not use.

Precautions While Using Dolono

It is very important that your doctor check the progress of you or your child while you are using this medicine. This will allow your doctor to see if the medicine is working properly and to decide if you or your child should continue to take it.

If your symptoms do not improve within a few days or if they become worse, check with your doctor.

Stop using this medicine and check with your doctor or dentist right away:

  • If you are taking this medicine to relieve pain, including arthritis pain, and the pain lasts for more than 10 days for adults or 5 days for children or if the pain gets worse, new symptoms occur, or the painful area is red or swollen. These could be signs of a serious condition that needs medical or dental treatment.

  • If you are taking this medicine to bring down a fever, and the fever lasts for more than 3 days or returns, the fever gets worse, new symptoms occur, or redness or swelling is present. These could be signs of a serious condition that needs treatment.

  • If you are taking this medicine for a sore throat, and the sore throat is very painful, lasts for more than 2 days, or occurs together with or is followed by fever, headache, skin rash, nausea, or vomiting.

Many combination medicines contain acetaminophen, including products with brand names such as Alka-Seltzer Plus®, Comtrex®, Drixoral®, Excedrin Migraine®, Midol®, Sinutab®, Sudafed®, Theraflu®, and Vanquish®. Adding these medicines to the medicine that you are already taking may cause you to get more than a safe amount of acetaminophen. Talk to your doctor before taking more than one medicine that contains acetaminophen.

Check with your doctor right away if you or your child have pain or tenderness in the upper stomach; pale stools; dark urine; loss of appetite; nausea; unusual tiredness or weakness; or yellow eyes or skin. These could be symptoms of a serious liver problem.

If you will be taking more than an occasional 1 or 2 doses of acetaminophen, do not drink alcoholic beverages. To do so may increase the chance of liver damage, especially if you drink large amounts of alcoholic beverages regularly, if you take more acetaminophen than is recommended on the package label, or if you take it regularly for a long time.

Taking certain other medicines together with acetaminophen may increase the chance of unwanted effects. The risk will depend on how much of each medicine you take every day, and on how long you take the medicines together. If your doctor or dentist directs you to take these medicines together on a regular basis, follow his or her directions carefully. However, do not take any of the following medicines together with acetaminophen for more than a few days unless your doctor has directed you to do so and is following your progress:

  • Aspirin or other salicylates

  • Diclofenac (e.g., Voltaren®)

  • Diflunisal (e.g., Dolobid®)

  • Etodolac (e.g., Lodine®)

  • Fenoprofen (e.g., Nalfon®)

  • Floctafenine (e.g., Idarac®)

  • Flurbiprofen, oral (e.g., Ansaid®)

  • Ibuprofen (e.g., Motrin®)

  • Indomethacin (e.g., Indocin®)

  • Ketoprofen (e.g., Orudis®)

  • Ketorolac (e.g., Toradol®)

  • Meclofenamate (e.g., Meclomen®)

  • Mefenamic acid (e.g., Ponstel®)

  • Nabumetone (e.g., Relafen®)

  • Naproxen (e.g., Naprosyn®)

  • Oxaprozin (e.g., Daypro®)

  • Phenylbutazone (e.g., Butazolidin®)

  • Piroxicam (e.g., Feldene®)

  • Sulindac (e.g., Clinoril®)

  • Tenoxicam (e.g., Apo-Tenoxicam®)

  • Tiaprofenic acid (e.g., Surgam®)

  • Tolmetin (e.g., Tolectin®).

Acetaminophen may interfere with the results of some medical tests. Before you have any medical tests, tell the person in charge if you have taken acetaminophen within the past 3 or 4 days. If possible, it is best to call the laboratory where the test will be done about 4 days ahead of time, to find out whether this medicine may be taken during the 3 or 4 days before the test.

For diabetic patients:

  • Acetaminophen may cause false results with some blood glucose (sugar) tests. If you notice any change in your test results, or if you have any questions about this possible problem, check with your doctor. This is especially important if your diabetes is not well-controlled.

For patients taking one of the products that contain caffeine in addition to acetaminophen:

  • Caffeine may interfere with the results of a test that uses adenosine (e.g., Adenocard®) or dipyridamole (e.g., Persantine®) to help find out how well your blood is flowing through certain blood vessels. Therefore, you should not have any caffeine for 8 to 12 hours before the test.

If you think that you or anyone else may have taken an overdose of acetaminophen, get emergency help at once, even if there are no signs of poisoning. Signs of severe poisoning may not appear for 2 to 4 days after the overdose is taken, but treatment to prevent liver damage or death must be started as soon as possible. Treatment started more than 24 hours after the overdose is taken may not be effective.

Dolacet

Pronunciation: a-SEET-a-MIN-oh-fen/HYE-droe-KOE-done
Generic Name: Acetaminophen/Hydrocodone
Brand Name: Examples include Dolacet and Stagesic

Dolacet contains acetaminophen. Severe and sometimes fatal liver problems, including the need for liver transplant, have been reported with the use of acetaminophen. Most cases of these liver problems occurred in patients taking excessive doses of acetaminophen (more than 4,000 mg per day). Also, patients who developed these liver problems were often using more than 1 medicine that contained acetaminophen. Discuss any questions or concerns with your doctor.

Dolacet is used for:

Treating moderate to moderately severe pain. It may also be used for other conditions as determined by your doctor.

Dolacet is an analgesic combination. Acetaminophen works by lowering a chemical in the brain that stimulates pain nerves. Hydrocodone, an opioid (narcotic) analgesic, works by binding to receptors in the brain and nervous system used by the body's natural "pain relievers." The combination may provide better pain relief than either medicine alone.

Do NOT use Dolacet if:

  • you are allergic to any ingredient in Dolacet

  • you have diarrhea associated with poisoning, antibiotic use, or a bacterial infection (from eating or drinking contaminated food or water)

  • you are taking sodium oxybate (GHB)

Contact your doctor or health care provider right away if any of these apply to you.

Before using Dolacet:

Some medical conditions may interact with Dolacet. Tell your doctor or pharmacist if you have any medical conditions, especially if any of the following apply to you:

  • if you are pregnant, planning to become pregnant, or are breast-feeding

  • if you are taking any prescription or nonprescription medicine, herbal preparation, or dietary supplement

  • if you have allergies to medicines, foods, or other substances

  • if you are allergic to other narcotic medicines (eg, medicines that contain codeine, morphine, oxycodone)

  • if you have or have ever had alcoholism, substance abuse, narcotic dependence, or if you consume more than 3 alcoholic drinks per day

  • if you have a history of suicidal thoughts or behavior

  • if you have a head injury, increased pressure in the head, a brain injury or tumor, epilepsy, or seizures

  • if you are having an asthma attack

  • if you have an acute stomach or intestinal problem, chronic inflammation and ulceration of the bowel, or you have had recent abdominal surgery

  • if you have adrenal gland problems; enlargement of the prostate gland; heart failure or lung disease, including shortness of breath; liver or kidney problems; thyroid disease; urinary blockage; or viral hepatitis

Some MEDICINES MAY INTERACT with Dolacet. Tell your health care provider if you are taking any other medicines, especially any of the following:

  • Barbiturate anesthetics (eg, thiopental), cimetidine, monoamine oxidase inhibitors (MAOIs) (eg, phenelzine), sodium oxybate (GHB), or tricyclic antidepressants (eg, amitriptyline) because serious side effects, such as increased sleepiness and decreased ability to breathe, may occur

  • Naltrexone because it may decrease the effectiveness of Dolacet and withdrawal symptoms may occur in patients who have become physically dependent on opioids

This may not be a complete list of all interactions that may occur. Ask your health care provider if Dolacet may interact with other medicines that you take. Check with your health care provider before you start, stop, or change the dose of any medicine.

How to use Dolacet:

Use Dolacet as directed by your doctor. Check the label on the medicine for exact dosing instructions.

  • Take Dolacet by mouth with or without food. If stomach upset occurs, take with food to reduce stomach irritation.

  • If you miss a dose of Dolacet, take it as soon as possible. If it is almost time for your next dose, skip the missed dose and go back to your regular dosing schedule. Do not take 2 doses at once.

Ask your health care provider any questions you may have about how to use Dolacet.

Important safety information:

  • Dolacet may cause drowsiness, dizziness, blurred vision, or light-headedness. These effects may be worse if you take it with alcohol or certain medicines. Use Dolacet with caution. Do not drive or perform other possibly unsafe tasks until you know how you react to it.

  • Do not drink alcohol or use medicines that may cause drowsiness (eg, sleep aids, muscle relaxers) while you are using Dolacet; it may add to their effects. Ask your pharmacist if you have questions about which medicines may cause drowsiness.

  • Dolacet may cause dizziness, light-headedness, or fainting; alcohol, hot weather, exercise, or fever may increase these effects. To prevent them, sit up or stand slowly, especially in the morning. Sit or lie down at the first sign of any of these effects.

  • Some of these products contain sulfites. Sulfites may cause an allergic reaction in some patients (eg, asthma patients). If you have ever had an allergic reaction to sulfites, ask your pharmacist if your product has sulfites in it.

  • Dolacet has acetaminophen in it. Before you start any new medicine, check the label to see if it has acetaminophen in it too. If it does or if you are not sure, check with your doctor or pharmacist.

  • Dolacet may harm your liver. Your risk may be greater if you drink alcohol while you are using Dolacet. Talk to your doctor before you take Dolacet or other fever reducers if you drink more than 3 drinks with alcohol per day.
